New Hires are required to complete the following documentation. Please click on the link for a PDF version of all required documents. Once all documents have been completed call the payroll office at 508-528-5400 x113 to make an appointment.
Employee Benefits (Optional) – Please complete the following documentation, depending upon which benefits you elect to participate in, before your appointment with the payroll office.
Current Employees can use the links provided under employee benefits for changing their elected benefits. Additionally, some forms (such as direct deposit forms) can be found in the required forms link under new hires above.
